Individual Player Waiver of Liability, Assumption of Risk, and Use of Likeness Release

All players MUST COMPLETE the CABL Individual Player Waiver of Liability, Assumption of Risk, and Use of Likeness Release. In order to be an eligible CABL player you must complete the “CABL Waiver” AND “Player Registration” forms.

1. Assumption of Risk *

I agree and give my permission

I understand that participation in baseball involves risks of serious injury, including but not limited to sprains, fractures, concussions, permanent disability, paralysis, or death. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of CABL, its officers, directors, volunteers, participants, or sponsors, and assume full responsibility for my participation.

2. Release of Liability *

I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless CABL, its officers, officials, agents, employees, volunteers, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct CABL activities (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

3. Medical Treatment *

I consent to receive medical treatment deemed necessary if I am injured during participation in CABL activities. I understand that I am solely responsible for all medical costs and expenses that may arise.

4. Use of Likeness *

I agree and give my permission

I hereby grant CABL and its authorized representatives the irrevocable right and permission to photograph, video record, and/or otherwise capture my name, image, voice, and likeness during CABL events. Such materials may be used for promotional, advertising, social media, or educational purposes without compensation. I waive any right to inspect or approve finished materials.

5. Compliance with Rules *

I agree and give my permission

I agree to abide by all rules, regulations, and codes of conduct established by CABL. I understand that failure to comply may result in suspension or removal from participation without refund.

6. Acknowledgment of Understanding *

I have read this Waiver and Release of Liability, fully understand its terms, and sign it freely and voluntarily. I understand that I am giving up substantial rights, including the right to sue.
